**I agree and understand that I MUST provide the following gear for this course:
- Long-sleeved shirt or jacket. Must cover all of your arms.
- Long jeans or sturdy pants without holes.
- Full fingered gloves. (No open back or cut out fingers)
- Sturdy boots/shoes that cover the ankle.
- D.O.T Helmet. (provided if needed)
- Shatter resistant eye protection. (i.e. sun glasses, prescription glasses, your helmets face shield)
- Valid photo ID / License.

| **I agree and understand that I MUST complete this course in its entirety. If I miss any segments of the course (this includes being tardy), I will not be able to continue and I will have to register and pay for a new class to receive a certificate.
